CYSA Adult Registration Requirement
ALL staff: Head Coaches, Assistant Coaches, and Trainers for Recreational, Camps, Comp, Select, and Tiered are required to submit a 1628 (CYSA Team Official Registration and Risk Management Disclosure) form to CYSA every year, usually before the Fall season. In addition, Team Managers who wish to have an Adult Pass for Comp and Select, or Team Managers who want the ability to sit on the player side of a game, or assist with games and/or practices, must also submit the 1628 form. All Tiered Team Managers must submit the 1628 form. Adults who do not submit the CYSA form will not be added to the CYSA roster, will not be granted a coaching pass, and will not be allowed to coach in any capacity in any program.
If a coach or team manager joins AVYSL during the middle of a season, or at the beginning of Spring season, he/she must submit the 1628 form before an adult pass will be granted.

In addition, each of the programs has its own application procedures for coaches and team managers as outlined below:
Comp/Select Grandfathered Teams’ Head Coaches and Assistant Coaches:
- Coaches are required to submit an AVYSL Coaches’ application every year.
- Along with this application, previous rosters, and current coaching license information must also be provided.
- This paperwork can be submitted to the Select or Comp Director, or to the AVYSL Registrar.
- Paperwork is generally submitted at the beginning of the year before the start of the Spring season.
- Team Managers are not required to submit this application.
Tiered Team Assistant Coaches:
- Every year, Assistant Coaches must notify their team’s Head Coach of their interest in coaching.
- Sign-ups are typically done during the initial Tiered Parent meeting
- Assistant Coaches must also submit the AVYSL Coaches’ application and current coaching license information every year.
- Team Managers are not required to submit this application for themselves.
- Paperwork for Assistant Coaches will be submitted to the AVYSL Registrar through the Tiered Team Manager as part of the team’s registration process.
Spring and Fall Recreation Head Coaches, Assistant Coaches, and Team Managers:
Recreation coaching and team management positions are assigned for one season at a time. Therefore, all parents interested in one of these positions must follow these steps:
- During the online registration process for players, parents are given an opportunity to identify how they would like to volunteer during the season. At this time, parents who are interested in one of these positions should sign up as Head Coach, Assistant Coach, and/or Team Manager. (Note that there is no Team Manager role during the Spring Recreational season).
- These parents will be contacted by the Spring or Fall Directors to discuss the role.
- If you decide you are interested in one of these roles after your child is already registered, please contact the AVYSL Registrar to update your registration record.
